Humana MA-PD HMO

The Humana Medicare Advantage and Prescription Drug (MA-PD) Employer HMO plan will be offered to the State of Florida Medicare-eligible retirees and their eligible dependents, effective January 1, 2020, as part of the State Group Insurance Program administered by the Florida Department of Management Services, Division of State Group Insurance.

Additionally, Humana Pharmacy received the highest score among mail order pharmacies in the J.D. Power 2018 U.S. Pharmacy Study of customers’ satisfaction with their pharmacy. 


The State of Florida Humana HMO Plan Highlights
The plan consists primarily of copayments and includes the following highlights: 

  • No plan deductible 
  • $1,500 medical out-of-pocket maximum 
  • $0 copay for Primary Care Physician office visits 
  • $10 copay for Specialist office visits
  • Copayments for prescriptions 
  • Pharmacy savings for a 90-day supply through mail order or at a retail pharmacy 
  • Routine hearing services 
  • Routine podiatry services 
  • Routine transportation services
  • Over the Counter (OTC) medication benefit

The State of Florida Humana HMO Plan Offers: 

  • All the benefits of Original Medicare, plus extra benefits 
  • Coverage for both medical and pharmacy benefits
  • One ID card for medical and pharmacy benefits 
  • Out of pocket maximum on medical benefits 
  • Your choice of an in-network provider to manage your care 
  • Large network of doctors, specialists, and hospitals to pick from 
  • Enhanced services, including member assistance, SilverSneakers fitness program, Go365 wellness program, Virtual Visits, Humana at Home, and WellDine meal program.
  • Access to 17 Humana in the Community locations throughout Florida that offer Humana members access to customer service support and daily activities and programming. 
  • MyHumana, a personal, secure online account that allows members to access their specific plan details from their computer or smartphone.
